Role of IL-18 in Comparison to Serum Creatinine in Early Detection of Sepsis Induced Acute Kidney Injury in Emergency Department in Suez Canal University Hospital

Background: Septic acute kidney injury is a syndrome of impairment function and organ damage linked with long-term adverse outcomes depending on the extent superimposed underlying reserve. Sepsis most important cause Acute Kidney Injury (AKI). Interleukin-18 (IL-18) pro-inflammatory cytokine expressed in renal cortex, peritubular capillaries, interstitium.
 Aim: To assess role IL-18 comparison to serum creatinine early detection sepsis-induced Emergency Department (ED) at Suez Canal University Hospital.
 Subjects Methods: A comparative cross-sectional study that included two groups participants. Study group: patient diagnosed attending ED Hospital. Control healthy individuals same age group. Patients were clinically assessed managed by ABCDE protocol. All patients subjected Initial assessment including History, clinical examination, laboratory investigation, urinary IL-8.
 Results: Cases had statistically significant higher compared controls (121.97 ± 75.84 vs 69.07 35.59) (p<0.001). IL-18, value 69.5 IU/L was found be best cut-off point for prediction AKI among cases, sensitivity = 6 5% specificity 57.5%.
 Conclusion: Urinary can used as an predictor than presenting sepsis.
